  1. Vraag 1 van 25

Why should you write notes as soon as possible after an event?
    Toon antwoord

Antwoord: D. Memory fades quickly, so early notes are more accurate

Details such as times, words and descriptions are lost fast. Notes made right away are the most reliable.

  2. Vraag 2 van 25

What kind of notebook is best for security work?
    Toon antwoord

Antwoord: D. A bound notebook with numbered pages

Bound, numbered pages show that nothing has been removed or added later.

  3. Vraag 3 van 25

You make a mistake in your notebook. What should you do?
    Toon antwoord

Antwoord: D. Draw a single line through it, initial it and write the correct information

A single line keeps the original readable, which shows you are not hiding anything.

  4. Vraag 4 van 25

May you tear a page out of your security notebook?
    Toon antwoord

    Antwoord: B. No, never

A missing page makes the whole notebook look unreliable, even if you had an innocent reason.

  5. Vraag 5 van 25

What should you do with unused space at the end of an entry?
    Toon antwoord

Antwoord: B. Draw a line through it so nothing can be added later

Closing off unused space shows that the entry was complete when you wrote it.

  6. Vraag 6 van 25

What should you write with in your notebook?
    Toon antwoord

Antwoord: D. A pen with permanent ink

Permanent ink cannot be changed without leaving a mark.

  7. Vraag 7 van 25

A person shouts a threat at you. How should you record it?
    Toon antwoord

Antwoord: B. Write their exact words in quotation marks

Exact words are strong evidence. Your summary or opinion is not.

  8. Vraag 8 van 25

An incident happens at 7:20 p.m. How do you write the time?
    Toon antwoord

    Antwoord: B. 19:20

    For times after noon, add 12 to the hour: 7 + 12 = 19.

  9. Vraag 9 van 25

Your notebook shows an entry at 00:45. What time is that?
    Toon antwoord

    Antwoord: B. 12:45 a.m. (just after midnight)

  10. Vraag 10 van 25

Ontario’s guard training syllabus lists basic report elements. Which is one of them?
    Toon antwoord

Antwoord: A. The time the report was completed

The syllabus lists date, time, location, actions or behaviours, description of individuals, observations and time of completion.

  11. Vraag 11 van 25

Which set of questions should a good incident report answer?
    Toon antwoord

Antwoord: B. Who, what, when, where, why and how

The 5 Ws and how make sure a reader gets the whole picture.

  12. Vraag 12 van 25

Which line from a report states a fact rather than an opinion?
    Toon antwoord

Antwoord: B. The man smelled of alcohol and was unsteady on his feet

Describe what you saw, heard and smelled. Let the reader draw conclusions from the facts.

  13. Vraag 13 van 25

What should you write in the “why” part of a report?
    Toon antwoord

Antwoord: D. The cause only if the facts show it, never a guess

Guessing a motive can be unfair and can damage your report. Stick to what the facts show.

  14. Vraag 14 van 25

Which is a type of report named in Ontario’s guard training syllabus?
    Toon antwoord

Antwoord: A. A use of force report

The syllabus names incident reports, use of force reports and witness statements.

  15. Vraag 15 van 25

    What is a witness statement?
    Toon antwoord

Antwoord: C. A person’s own account of what they saw, in their own words

A statement records the witness’s account, not yours. The witness reads and signs it.

  16. Vraag 16 van 25

When taking a witness statement, what is good practice?
    Toon antwoord

Antwoord: C. Let the witness tell it in their own words, then have them read and sign it

Witnesses who hear each other can mix up what they saw. Keep each account separate and in the person’s own words.

  17. Vraag 17 van 25

In what order should you write the events in a report?
    Toon antwoord

Antwoord: C. In time order, from first to last

Time order makes the report easy to follow and matches your notebook.

  18. Vraag 18 van 25

Why should you avoid site codes and slang in a report?
    Toon antwoord

Antwoord: C. Readers who were not there may not understand them

Police, lawyers and insurers may read your report. Plain words mean the same thing to everyone.

  19. Vraag 19 van 25

Why must your report be accurate and complete?
    Toon antwoord

Antwoord: A. It may be used for audits or as evidence in court

Ontario’s syllabus notes that reports may be needed for audits or as evidence in court.

  20. Vraag 20 van 25

What is the safest way to send a confidential incident report?
    Toon antwoord

Antwoord: C. Through your employer’s approved system, only to authorized people

Ontario’s syllabus warns about problems with emailing confidential reports. Reports often contain personal information.

  21. Vraag 21 van 25

Which part ends every notebook entry?
    Toon antwoord

    Antwoord: D. Your signature

Signing shows the entry is yours and is finished.

  22. Vraag 22 van 25

You forgot to record something earlier in the shift. What should you do?
    Toon antwoord

Antwoord: B. Add it as a late entry with the current time and say it is late

A clearly marked late entry is honest. Squeezing notes between lines looks like tampering.

  23. Vraag 23 van 25

What should you do before handing in a report?
    Toon antwoord

Antwoord: B. Read it again and check that it matches your notes

A final check catches errors and keeps your report consistent with your notebook.

  24. Vraag 24 van 25

Which description of a person is most useful in a report?
    Toon antwoord

Antwoord: D. About 180 cm tall, thin, short black hair, red jacket, blue jeans, white running shoes

Specific, factual details help others find and identify the person.

  25. Vraag 25 van 25

Who should you picture as the reader of your report?
    Toon antwoord

Antwoord: B. Someone who was not there and knows nothing about the event

If a stranger can understand the report without asking questions, it is clear enough.

Veelgestelde vragen

Can I use my notebook in court?

Often, yes. A judge may let you refresh your memory from notes you made close to the event. That is why notes must be complete, neat and made as soon as possible.

What if I make a mistake in my notebook?

Draw a single line through it so it can still be read, write your initials, and write the correct information. Never erase, use correction fluid or tear out a page.

Should I put my opinion in an incident report?

No. Write what you saw, heard and did. Instead of “he was drunk”, describe the signs: the smell of alcohol, slurred speech, trouble standing.

What is the difference between a statement and a report?

A report is your own account as the guard. A statement is another person’s account of what they saw, in their own words, which they read and sign.
