  2. Question 2 of 25

Ontario's Regulation 851 says a lift truck operator must be what?
    Show answer

    Answer: B. A competent person

Clause 51(2)(a) of Regulation 851 (Industrial Establishments) requires the operator to be a competent person.

  3. Question 3 of 25

In BC, lift trucks and their operator training must meet which standard?
    Show answer

Answer: A. CSA B335, Safety standard for lift trucks

BC's OHS Regulation s. 16.43 requires lift trucks to meet CAN/CSA B335-15, and operator training to meet Clause 6 of that standard.

  4. Question 4 of 25

Ontario's lift truck guideline says employers should keep a record for each competent operator. What should it show?
    Show answer

Answer: B. Truck class, assessor and date

The record should show the skills and knowledge shown, the truck class or classes assessed, the assessor's name and affiliation, and the date.

  5. Question 5 of 25

When should a forklift get its daily check?
    Show answer

Answer: B. At the start of the shift

Forklifts should be inspected every day, at the start of each shift and before use. BC requires the operator to inspect mobile equipment before it is first used on a shift.

  6. Question 6 of 25

During your pre-use check you find the horn does not work. What should you do?
    Show answer

Answer: C. Report it to your supervisor right away

Problems found in the daily check should be reported to the supervisor immediately. Do not make repairs unless you are authorized.

  7. Question 7 of 25

What is the first part of a daily forklift check, done before you start the engine?
    Show answer

    Answer: D. A visual walk-around check

The daily check starts with a visual (circle) check before starting, followed by an operational check with the key on.

  8. Question 8 of 25

Where do you find the maximum load a forklift can lift?
    Show answer

Answer: A. On the truck's data plate

Each lift truck has a maximum load limit, shown on its data plate. Never exceed it.

  9. Question 9 of 25

What happens to a forklift's safe load limit as the load is raised higher?
    Show answer

    Answer: D. It goes down

The load limit decreases as the load centre is raised, and raising a load makes the truck less stable.

  10. Question 10 of 25

An uneven load is heavier at one end. How should you pick it up?
    Show answer

Answer: D. Heavier end closest to the front wheels

Keep the heavier end nearest the front wheels and tilt the mast back.

  11. Question 11 of 25

The forks should reach at least how much of the load's length?
    Show answer

    Answer: A. Two-thirds

CCOHS says forks should be at least two-thirds of the load length and inserted fully under the load.

  12. Question 12 of 25

How high should the forks be when you travel with a load?
    Show answer

Answer: D. About 10 to 15 cm off the floor

Travel with the forks as low as possible and tilted back, usually about 10 to 15 cm (4 to 6 in.) above the floor.

  13. Question 13 of 25

You drive a loaded forklift up or down a steep ramp. Which way should the load point?
    Show answer

    Answer: A. Uphill

On grades over 5%, a loaded truck keeps the load pointed uphill, both going up and coming down.

  14. Question 14 of 25

On a ramp with no load, which way should the forks point?
    Show answer

    Answer: C. Downhill

Unloaded trucks keep the forks pointed downhill on any grade.

  15. Question 15 of 25

Where should you avoid turning a forklift?
    Show answer

    Answer: B. On a ramp

Do not turn on ramps. Turn on level ground whenever possible.

  16. Question 16 of 25

A co-worker asks for a ride on your forklift. What should you say?
    Show answer

Answer: C. No, unless it is built for a passenger

Nobody but the operator should ride on the truck unless it is designed for a second person.

  17. Question 17 of 25

If your forklift starts to tip over, what does CCOHS advise?
    Show answer

Answer: B. Stay in the truck

CCOHS advises operators to stay in the truck in case of overturn, and to wear the seat belt or restraint when one is fitted.

  18. Question 18 of 25

Is it ever safe to stand or walk under a forklift's raised forks?
    Show answer

Answer: D. No, never stand or walk under them

No one should stand or pass under the elevated part of a forklift, loaded or unloaded.

  19. Question 19 of 25

You are on foot and need to cross an aisle where a forklift is working. What should you do?
    Show answer

Answer: C. Make eye contact with the driver first

Pedestrians should use the walking areas set aside for them and make eye contact with the driver before crossing.

  20. Question 20 of 25

What is the best way to keep pedestrians safe from forklifts?
    Show answer

Answer: A. Separate walkways and barriers

Traffic management separates people and trucks with marked walkways, exclusion zones and physical barriers such as bollards or fences.

  21. Question 21 of 25

What is commonly installed at blind intersections in a warehouse to help drivers and pedestrians see each other?
    Show answer

    Answer: C. Convex mirrors

Convex mirrors and warning lights at intersections help prevent collisions.

  22. Question 22 of 25

Before a forklift drives into a trailer at a loading dock, what must be done?
    Show answer

Answer: D. Chock the trailer's wheels

Make sure the truck brakes are set and the wheels are chocked, or use a trailer restraint, so the trailer cannot move away from the dock.

  23. Question 23 of 25

Why are sparks, flames and smoking banned where forklift batteries are charged?
    Show answer

Answer: C. Charging gives off explosive hydrogen

Charging lead-acid batteries releases hydrogen gas, which can explode, so charge only in approved, ventilated areas away from ignition sources.

  24. Question 24 of 25

Under BC's rules, what must you do before leaving a forklift's controls unattended?
    Show answer

Answer: C. Secure it and lower any raised load

BC's OHS Regulation s. 16.9 requires mobile equipment to be secured against movement, and elevated loads to be landed or secured, before it is left unattended.

  25. Question 25 of 25

A worker needs to be lifted on a forklift to reach a high shelf. What is required?
    Show answer

Answer: A. A platform made for lifting people

People may be raised only on a work platform built or certified for that purpose, secured to the forks, with guardrails, and with the operator at the controls.
