OSHA Forklift Operator Practice Test 2026

In the US, OSHA requires employers to train, evaluate and certify every forklift operator under 29 CFR 1910.178. There is no OSHA forklift license or national exam: your employer, or a trainer it uses, gives formal instruction, practical training and an evaluation of your driving at work. This independent practice covers the topics the standard and OSHA's forklift eTool describe.

What to know before the test

  • Only trained and competent operators may run a forklift. Training combines formal instruction, practical training and an evaluation of the operator's performance in the workplace.
  • Each operator's performance must be evaluated at least once every three years.
  • Refresher training is required after unsafe operation, an accident or near-miss, a poor evaluation, a move to a different type of truck, or a workplace change that affects safe operation.
  • The employer certifies each operator with the operator's name, training date, evaluation date and the name of the trainer or evaluator.
  • Federal child labor rules (Hazardous Occupations Order No. 7) ban workers under 18 from operating forklifts in nonagricultural jobs.
  • Forklifts must be examined at least daily before use, and after each shift when used around the clock.
  • OSHA says forklift overturns are the leading cause of fatalities involving forklifts.

Frequently asked questions

Is there an OSHA forklift license?

No. OSHA does not issue forklift licenses. Your employer must train and evaluate you and then certify that you are competent to operate the type of truck you will use.

How long is forklift certification good for?

OSHA requires an evaluation of each operator at least once every three years. You need refresher training sooner after unsafe driving, an accident or near-miss, or a move to a different type of truck.

Can I take an online forklift course to get certified?

Online lessons can cover the formal instruction part only. The standard also requires practical training and an evaluation of your driving in the workplace, done by a qualified person.

How old do you have to be to drive a forklift at work?

In nonagricultural jobs, federal child labor rules ban workers under 18 from operating, riding on or helping to operate forklifts.

Does my certification transfer to a new job?

Each employer must certify its own operators. A new employer may skip topics you were already trained on if that training fits its trucks and workplace and you are evaluated as competent.
